Home and Community Support
The Home and Community Support program is designed to meet the needs of children, birth through age twenty, who have a specific diagnosis of Mental Retardation, Pervasive Development Disorder or Autism. Broadreach Home and Community Support is FREE to eligible children.
The Home and Community Support program assists enrolled children in meeting identified goals that focus primarily on behavior management and physical development through the provision of individualized support in home and community settings.
We work as a team with families and other providers to develop and implement an individualized Behavioral Treatment Plan that addresses the child’s sensory, motor and psychological needs and promotes
- self-maintenance,
- physical development and fitness,
- self-awareness, and
- self motivation.
Our Habilitation Specialists work with families and other providers as part of the Child and Family Team; provide one on one support to children in their homes, or at other community settings in order to assist the child and family in meeting identified goals; provide opportunities and support for the child to integrate in his/her community and interact with peers; observe and record the child’s progress towards individualized goals; are available when help is needed; have flexible hours to accommodate each family’s needs; and
provide services as long as a child is eligible, and the family chooses them.
Broadreach Family & Community Services
has been authorized by the Maine Department of Behavioral and Developmental Services to deliver Home and Community Support services. These services are paid for by Maine Care.
